This Testimonial Consent and Release (“Agreement”) is entered into by the undersigned individual (“Client”) in favor of Shore Law, LLC, an Illinois limited liability company, including its attorneys, employees, agents, representatives, successors, and assigns (collectively, “Shore
Law”).
1. Authorization to Use Testimonial and Likeness
Client voluntarily authorizes Shore Law to photograph, film, record, and/or otherwise capture Client’s name, voice, image, likeness, statements, testimonial, and/or case-related comments (collectively, the “Materials”).
Materials may include, without limitation:
- Client’s personal testimonial, in whole or in part
- Written or recorded statements regarding legal services received
- General description or summary of Client’s matter
- Client’s first name and last initial, city, and state
- Photographs and/or video recordings
Client understands that participation is entirely voluntary.
2. Permitted Use
Client grants Shore Law the irrevocable, perpetual, royalty-free right to use, reproduce, edit, publish, distribute, display, and otherwise use the Materials for lawful marketing, advertising, educational, promotional, and informational purposes.
Such use may include, but is not limited to:
- Shore Law’s website(s)
- Social media platforms
- Print and digital advertising
- Email communications
- Video platforms
- Legal directories
- Presentations or marketing materials
- Media publications
Shore Law may edit the Materials for clarity, length, formatting, or production quality, provided that the substance of the testimonial is not materially altered.
3. No Compensation
Client understands and agrees that no compensation has been promised or provided in exchange for this testimonial unless separately disclosed in writing.
Client further understands that providing a testimonial does not affect the status of any past, present, or future legal representation.
4. No Guarantee of Results
Client acknowledges that:
- The testimonial reflects Client’s individual experience.
- Past results do not guarantee future outcomes.
- Every legal matter is different and depends on its own facts, evidence, parties, and applicable law.
Client agrees that Shore Law may include appropriate disclaimers consistent with professional responsibility rules.
5. Confidentiality and Voluntary Disclosure
Client understands that by providing a testimonial, Client may voluntarily disclose information about Client’s legal matter.
Client confirms that:
- Client is voluntarily choosing to share such information.
- Shore Law has not required disclosure of confidential information.
- Client understands that once published, information may be publicly accessible.
Client releases Shore Law from any claims related to the voluntary disclosure of such information through the Materials.
6. Right to Revoke Future Use
Client may request in writing that Shore Law discontinue future use of the Materials. Shore Law will make reasonable efforts to remove the Materials from future publications within its control; however, Client understands that materials already published, distributed, or archived may not be fully retrievable.
7. Release of Claims
Client releases and holds harmless Shore Law from any and all claims, demands, damages, liabilities, or causes of action arising from the authorized use of the Materials, including but not limited to claims for invasion of privacy, defamation, false light, or misappropriation of likeness.
8. Age and Capacity
Client affirms that:
- Client is at least 18 years of age OR
- Client is the parent/legal guardian of a minor and has authority to execute this Agreement on
the minor’s behalf.
Client further affirms that Client is legally competent to execute this Agreement.
9. Entire Agreement
This Agreement constitutes the entire understanding between Client and Shore Law regarding the subject matter herein and supersedes any prior discussions or agreements related to testimonial use.
